| The Malaysian AIDS Foundation is
again holding the biennial Dr Siti Hasmah Award this year
in recognition of outstanding contributions made by organisations
and individuals in addressing HIV/AIDS in Malaysia. |
| |
| Recognising excellence and long-term
commitment to work having a direct impact on HIV/AIDS,
the award is an invaluable avenue to celebrate and |
|
|
|
| encourage more organisations and individuals
to be more proactive, creative and committed in their response
to HIV/AIDS. |
| |
| The MAF welcomes nominations for organisations
or individuals whose efforts and/or contributions cover any
one or more of the following areas of work: |
- Raise awareness of HIV/AIDS issues in Malaysia
- Prevent the spread of HIV in Malaysia
- Support People Living with HIV/AIDS in Malaysia (including
treatment and care programmes)
- Involvement with community, local or national efforts
to counter HIV/AIDS that help alleviate the impact of the
epidemic in Malaysia
- Recognising the importance of vulnerable groups such as
children, youth and women as well as marginalised communities
and addressing the needs of these groups in Malaysia

| The award is given to contributions made
between March 1, 2006, and February 29, 2008.
Nominees must be a Malaysian citizen or organisation based in
Malaysia. |

| The winner will receive a specially designed
trophy, sponsored by Mondial, cash prize of RM10,000 (organisation)
and RM5,000 (individual), and a Certificate of Excellence. These
will be presented on October 25, 2008, at the Mandarin Oriental
Kuala Lumpur, in conjunction with the Red Ribbon Gala. |

| Eligibility of Nominees |
- An individual who is a Malaysian citizen or an organisation
based in Malaysia;
- An individual or organisation cannot nominate themselves.
Nominations must be made by a third party
- Contributions by the nominee must have been made between
the period of 1 March, 2006, and 29 February, 2008

| The Judges' Criteria |
| Nominations will be judged based on: |
- A clear rationale for the nominee's involvement in HIV/AIDS
work;
- Evidence of, or evaluation of a benefit for the targeted
community;
- Any attempts to sustain and/or share the benefits of learning.

| What to send in: |
| Nominations may be submitted in English
or Bahasa Malaysia and should include: |
- One copy of the nomination form, duly completed;
- Rationale with a brief biography or organisation sketch
outlining salient points as to why the nominee should be
considered for the award (maximum two pages A4 size paper).
|
| The rationale should provide specific examples
explaining how the nominee has demonstrated outstanding commitment
to work having a direct impact on HIV/AIDS. |
| |
| Entries without a rationale will be immediately
disqualified for review by the panel of judges. |
| |
| Notification |
| The MAF will contact all nominees to assess
if they have objections to being nominated. Short listed nominees
will be invited for an interview with the panel of judges. |
